For a successful DOT/CDL physical, it’s important to come prepared with all required paperwork and take any necessary steps before your visit. To make the process as efficient as possible, please ensure you bring and do the following:
You may need to bring additional documentation if you have any of the following health issues:
*All drivers who have been granted a FMCSA-issued medical waiver must present their waiver form at their physical examination. If drivers have additional health issues or recent surgical procedures, proper documentation may be required at the time of being seen. If the necessary documentation is not provided, our physicians may not certify the driver during that visit. In this case, the driver may be asked to return at a later date, for an additional fee.
Your provider at AFC Urgent Care Arden will review the following during your physical:
A Department of Transportation (DOT) physical can be valid for up to two years, though the exact period of validity may depend on the person’s health conditions and any medical issues that would prevent them from adequately carrying out safety-sensitive tasks.
If the medical examiner decides that you fulfill the physical requirements to drive a commercial motor vehicle, they will provide a copy of the results to you and fill out a Medical Examiner’s Certificate. It is then the patient’s responsibility to provide copies of this certificate to their employer.
*If a patient fails their examination due to misrepresentation or a disqualifying medical condition, they are still obligated to pay for their DOT physical.
