We want to ensure that you feel fully prepared when you come to our clinic. For a smooth experience, please bring the following items:

● Insurance Card – only if you’re paying with insurance
● Valid Photo Identification
● Co-Pay – cash or credit card

1. Check In
Once you arrive, please make your way to our reception desk where you will be asked to fill out a registration form (if this is your first visit). After completing the form, you can wait in our comfortable lobby until you are seen.

2. Medical Exam
Our medical personnel will escort you to one of our examination rooms, where you can present a more comprehensive description of your condition or injury. After that, your provider will determine any necessary testing or treatment.

3. Aftercare
After treating the cause of your visit, your health care provider will likely suggest a treatment plan or a prescription to get you feeling normal again. We will provide you with complete aftercare instructions, including an excuse letter for work or school if necessary.
