DOT Physical FAQs for Drivers

What are DOT Physicals?

Department of Transportation Physicals, commonly referred to as DOT Physicals, are medical exams to determine if truck drivers and other commercial vehicle operators are healthy enough to drive.

A DOT Physical is required before a commercial drive can renew their Commercial Driver’s License (CDL). Truck drivers that don’t take the time to complete their CDL may experience delays or gaps in their driving schedule.

Unless drivers have a specific exemption or waiver, they are required to undergo medical physicals based on federal medical guidelines. Additionally, each state requires drivers to meet other qualifications in order to get their physical completed.

According to Massachusetts’ standards for CDL renewal, a driver must meet these federal medical requirements to pass their DOT Physical:

  • Hearing: Ability to hear a forced whisper from more than 5 feet away in at least 1 ear.
  • Vision: Correctable 20/40 vision in each eye.
  • Cardiac Health: No chronic conditions that put the drive at risk for heart attack or COPD, unless medication suggests the condition is under control.
  • Diabetes: Requires a stable blood sugar level and effective management of diabetes. Having diabetes does not immediately disqualify your DOT physical.
  • Blood Pressure: 140-159 / 90-99 = 1 yr certificate; 160-179 / 100-109 = 3 mo temp certificate based on examiner’s discretion; >180 / 110 = Disqualified for CDL renewal
