Immigration Physicals

Southcenter Immigration Physicals at AFC Urgent Care

AFC Urgent Care Southcenter is now qualified to perform immigration physicals that meet the requirements set by the Department of Homeland Security (DHS) and US Citizen and Immigration Services (USCIS) for individuals and families applying for citizenship, permanent residency or green card status in the United States. Each examination follows strict protocol put forth by the DHS, USCIS and the CDC. Immigrations are performed by appointment only when Civil Surgeon is on duty. The examination is a requirement of the application process for citizenship. AFC Urgent Care Southcenter is able to perform any necessary bloodwork and x-rays needed as part of your physical.

What Should I Expect At My Immigration Physical?

Two appointments will be needed to complete your immigration physical. Please arrive at your appointment early to ensure that all paperwork is completed and correct. At your first appointment, you will undergo Routine blood work and have your I-693 form filled out. You will need to provide a copy of your medical history, which will be reviewed during your physical. During your second appointment, you will meet with our Civil Surgeon and undergo a physical. Please expect to have at least two weeks between your visits in order for lab results to come in. The following will be done over the course of your visits:

  • Physical Exam
  • Mental Evaluation
  • Review Of Medical And Immunization History
  • Chest X-Ray (If Necessary)
  • Evaluation Of Any Physical Or Mental Disability Or Disease

What To Bring To Your Immigration Physical

You will need to bring the following to your immigration physical:

  • Bring your own interpreter or ask how to arrange one prior to the visit.
  • Government issued identification (driver’s license, passport, government ID card) for all family members, including children
  • Translated immunization records
  • Hospital records, including proof of potential tuberculosis treatment
  • Previous chest x-rays
  • Records of previous mental health treatment
  • I-693 form

  • How long will the exam take?

    AFC offers some of the lowest wait times around, with many patients getting in to see a provider in less than 15 minutes. Your exam will probably take around a half hour.

  • What happens if my physical reveals I have a medical problem?

    We’ll complete your paperwork as required, but if it’s something we can help with we might be able to provide you with treatment on the spot. With in-house labs at many locations, you could be on your way without the need for a second visit.
