Southcenter Immigration Physicals at AFC Urgent Care

AFC Urgent Care Southcenter is now qualified to perform immigration physicals that meet the requirements set by the Department of Homeland Security (DHS) and US Citizen and Immigration Services (USCIS) for individuals and families applying for citizenship, permanent residency or green card status in the United States. Each examination follows strict protocol put forth by the DHS, USCIS and the CDC. Immigrations are performed by appointment only seven days a week. The examination is a requirement of the application process for citizenship. AFC Urgent Care Southcenter is able to perform any necessary bloodwork, vaccinations and x-rays needed as part of your physical.

What Should I Expect At My Immigration Physical?

Two appointments will be needed to complete your immigration physical. Please arrive at your appointment early to ensure that all paperwork is completed and correct. At your first appointment, you will undergo Routine blood work and have your I-693 form filled out. You will need to provide a copy of your medical history, which will be reviewed during your physical, as well as vaccination history. During your second appointment, you will meet with our Civil Surgeon, undergo a physical and receive any vaccinations needed. Please expect to have at least two weeks between your visits in order for lab results to come in. The following will be done over the course of your visits:

• Physical Exam
• Mental Evaluation
• Review Of Medical And Immunization History
• Chest X-Ray (If Necessary)
• Evaluation Of Any Physical Or Mental Disability Or Disease
• Vaccination

What To Bring To Your Immigration Physical

You will need to bring the following to your immigration physical:

• Government issued identification (driver’s license, passport, government ID card) for all family members, including children
• Translated immunization records
• Hospital records, including proof of potential tuberculosis treatment
• Previous chest x-rays
• Records of previous mental health treatment
• I-693 form
