What to Bring to your DOT/CDL Physical

Dot physical exams require certain documentation. Come prepared for your physical so the process can be as simple as possible. Please bring and do the following to prepare for your visit:

• Driver’s License
• Hearing aids with extra batteries
• Glasses (If you require correction to your vision, whether or not you wear contacts, you must bring a pair of glasses)
• Full bladder (must provide a urine sample)
• Complete medication list from your pharmacy with dosage amounts
• Wear loose clothing
• Continue taking any prescribed medication

You may need to bring additional documentation if you have any of the following health issues:

• Heart issues
• Diabetes/Pre-diabetes
• Hypertension (or if you’re on hypertensive medication)
• Sleep apnea
• Respiratory disorders
• Smokes 35 years or older
• Neurological disorders
• Mental or behavioral health issues
• Blood thinners
• Vision issues
• Controlled substances

*All drivers granted a FMCSA-issued medical waiver must have their waiver form in hand for their physical examination. In cases of additional health issues or recent surgical procedures, additional information may be required. Should proper documentation not be supplied, we may not be able to certify the driver at their visit. You may be asked to return at a later date for an additional charge.

How long is my DOT physical valid?

A DOT physical is valid for up to 24 months. However, the specific length of time that a DOT physical is valid can vary depending on the individual’s specific health conditions and any medical conditions that may disqualify them from performing safety-sensitive duties.

What happens after my DOT physical?

If the medical examiner finds that you are physically qualified to drive a commercial motor vehicle, the medical examiner will furnish one copy of the results to you and complete a Medical Examiner’s Certificate. It is the patient’s responsibility to provide copies of this certificate to your employer.

*If you fail to pass your exam due to misrepresentation or a disqualifying medical condition, you will still be responsible for paying for your DOT physical.
