Immigration Physicals in Portland, OR
Applying for a green card or permanent residency? AFC Urgent Care Portland completes the medical exam USCIS requires, start to finish. Our USCIS-designated civil surgeons follow the guidelines set by the Department of Homeland Security, USCIS and the CDC, and record your results on Form I-693.

How Your Immigration Physical Works
Plan on at least two visits. Please arrive 30 minutes early to the first one to fill out paperwork.
Visit 1: Paperwork and testing
A medical assistant reviews your documents, records them on your I-693 and starts the required testing, including blood work.
Visit 2: Civil surgeon exam
About 7 days later, once your lab results are back, the civil surgeon reviews your health history, completes a full physical exam and orders any vaccinations you still need. If your civil surgeon is male, a female staff member will be in the room for female patients.
Signing your forms
Once every report is in and reviewed, we will let you know your documents are ready to sign. Extra visits are occasionally needed depending on test results.

What to Bring to Your Appointment
- Government-issued photo ID, such as a driver's license or passport. Children need their own photo ID; a parent's ID will not be accepted.
- Vaccine and immunization records, translated into English if they are in another language (uscis.gov/i-693)
- Hospital or clinic records, including proof of any prior treatment for tuberculosis or other infectious diseases
- Prior chest x-rays or other diagnostic tests
- Any mental health records
- All correspondence you have received from USCIS
- Payment (cash or credit card)
Need an interpreter? You are welcome to bring your own. We can also provide a certified medical interpreter with two days' notice for an additional fee.

How long does an immigration physical take at AFC Portland?
Plan on at least two visits. The first covers paperwork and testing, and the civil surgeon exam is scheduled about 7 days later so your lab results are back in time. Arrive 30 minutes early to your first visit.

Yes. Our USCIS-designated civil surgeons perform the exam according to DHS, USCIS and CDC guidelines and record your results on Form I-693 for you to submit with your application.

Can I get my required vaccines and chest x-ray at AFC?
Yes. Labs, chest x-rays and required vaccinations are all available on site, so you can complete the medical part of your application in one place. Vaccines are billed separately.
