What are your hours of operation?

We're open Monday through Friday from 8:00 AM to 8:00 PM, and Saturday and Sunday from 8:00 AM to 5:00 PM.

What is your fax number?

Our fax number is 781-848-2275. If you're an adjuster or occupational medicine provider looking for workers' comp paperwork or results, please call us and we'll be happy to assist with your specific request.

Do you offer DOT physicals?

Yes! DOT physicals are available on a walk-in basis anytime during our regular hours — no appointment needed. Just be sure to bring a valid driver's license.

Do you have X-ray services on-site?

Yes, we offer on-site X-ray services during all regular business hours.

How do I request my medical records?

Medical records requests are handled by HealthMark. Simply fax your request to 205-578-6090 and they will process it for you.

Who do I contact with billing questions?

For any billing questions, please contact our billing department directly at 413-540-0150 or 413-887-6323.

Do I need an appointment?

No appointment necessary! We see patients on a walk-in basis anytime during our regular hours.

How do I get my lab results?

Give us a call and we'll check whether your results are in. If they're ready, a member of our medical team will go over them with you.

Where are you located? Is there public transportation nearby?

We're conveniently located at 45 Drum Hill Rd, Chelmsford, MA 01824, between the South Shore Plaza and Five Corners, in the plaza with Supercuts and GEICO. There is a bus stop nearby for easy access via public transportation.
