  • Hearing aids and extra batteries (if applicable)
  • Glasses (even if you wear contacts)
  • Full bladder for the urine sample
  • Complete medication list with dosages
  • Medical forms if you have specific health conditions

What to Know about Your DOT Physical

DOT physicals are valid for up to 24 months. We can accommodate drivers with specific health conditions, but additional forms or information may be required. Please note that failing a physical due to misrepresentation or disqualifying medical conditions will still incur a charge.
