We want you to feel prepared when you visit our clinic. If you’re not sure what is required, bring the following items for a smooth and efficient experience:

• Insurance Card – only if you’re paying with insurance
• Valid Photo Identification

1. Check In

When you arrive, head inside to our welcoming reception desk. to complete a short questionnaire. An AFC team member will ask you to complete a registration form (if you haven’t seen us before). Once you complete your form, you may wait in our comfortable lobby until your turn to be seen.

2. Medical Exam

One of our medical personnel will take you into one of our examination rooms where you can explain your illness or injury in more detail. From there, your provider will determine any necessary testing or treatment.

3. Aftercare

Once we have treated the reason for your visit, your provider may suggest a treatment plan or prescribe medication to get you feeling back to normal. We also provide you with complete aftercare instructions and an absentee note for work or school if needed.
