AFC Urgent Care & Walk-In Clinic in West Chester, PA, has an Excellent Staff of Board-Certified Physicians,
Physician Assistants, Nurse Practitioners, X-ray technicians, and Medical Assistants.
Our highly trained staff have family medicine, emergency medicine, and acute care backgrounds, and are ready to
address your medical needs. We are a professional, compassionate, and caring team. Our urgent care center in West
Chester contains a full lab and advanced medical technology to ensure you receive the fastest and most accurate
treatment possible. Our goal is to bring you the best possible medical care and get you back on your feet quickly.

Our Providers
Leonard Sax, MD, PhD, FAAFP
Dr. Sax graduated Phi Beta Kappa from the Massachusetts Institute of Technology (MIT) at the age of 19. He then
earned both a PhD in psychology and an MD, at the University of Pennsylvania. After completing a three-year
residency in family medicine at Lancaster General Hospital, Dr. Sax launched his own medical practice in Montgomery
County Maryland, just outside Washington, DC, where he practiced for 18 years. He has worked at our center since its
launch in October 2015. He is also a DOT-certified medical examiner. Dr. Sax is the author of four books for
parents, most recently The Collapse of Parenting, which was a New York Times bestseller. He lives with his wife and
daughter near Chester Springs.
Amanda D. Miller, DO
While not originally from Pennsylvania, Dr. Miller has made the Philly area her home since 2002. She earned her
undergraduate degree in Biology & Chemistry from the University of Southern Mississippi, then went on to receive her
medical degree from the Kansas City University School of Osteopathic Medicine. After completing an internship year
with the Philadelphia College of Osteopathic Medicine, she did her Residency training in Emergency Medicine at
Thomas Jefferson University Hospital in Philadelphia. She has over 20 years of experience as an attending physician
in Emergency Medicine, working at several ERs in the tri-county area. As a certified Civil Surgeon, she conducts
Immigration medical exams here at AFC, and works part-time as a certified Physician Advisor for healthcare systems
and companies.
Dr. Miller is passionate about improving the quality & accessibility of healthcare. She lives in Chester County
with her husband, kids, 2 dogs and 2 guinea pigs
Oliver Thresher, Jr., MD
Dr. Thresher is a Pennsylvania native who grew up in Philadelphia. He graduated from Syracuse University with a
bachelor’s degree in biology and earned his medical degree from St. George’s University. After completing his
residency at Delaware County Memorial Hospital, Dr. Thresher opened a private practice in Rising Sun, MD, where he
practiced primary care for 25 years. Dr. Thresher has spent the last 15 years practicing in various urgent care
centers across Delaware, Maryland, and Pennsylvania. He is board-certified in family medicine, a DOT-certified
medical examiner, and a Civil Surgeon who can perform immigration physicals. He lives in Chester County and enjoys
spending time with his family and dogs, as well as relaxing at the beach during the summer months.
Vicky Kutch, DNP
Dr. Vicky is a board-certified Family Nurse Practitioner who is passionate about providing patient-centered care.
She earned her Bachelor of Science in Biology from Salisbury University in 2012, followed by her Master of Science
in Nursing from the University of Maryland, Baltimore, in 2015 through an accelerated registered nurse program. Dr.
Vicky spent eight years as a registered nurse, working in diverse settings including ICU, ICU step-down, and
research. In 2022, she earned her Doctor of Nursing Practice. She then completed a one-year Nurse Practitioner
residency at the Durham, North Carolina, VA in partnership with Duke University. Since then, she has focused her
practice on both primary care and urgent care medicine. Dr. Vicky is also a DOT-certified medical examiner. Outside
of the clinic, Vicky enjoys traveling, hiking, and exploring the outdoors. She’s also an animal lover and shares her
home with her two cats, Binx and Dani.
Sierra Mazzarella, MSN, RN, FNP-C
Sierra is a Certified Registered Nurse Practitioner. She completed her Master's of Nursing at DeSales University.
Prior to that, Ms. Mazzarella was working as a registered nurse at Reading Hospital. She completed her Bachelor's of
Nursing at Indiana University of Pennsylvania. She is also a DOT-certified medical examiner. In her free time,
Sierra likes to travel, read, and spend time with her husband and three dogs. Sierra started working in healthcare
because of her passion for helping others achieve their health goals.
Rebecca Nguyen, PA-C
Becca grew up in Downingtown and is the proud daughter of Vietnamese immigrants. She graduated at the top of her
class from Temple University and went on to earn her master’s degree from St. Joseph’s University, becoming a
certified physician assistant. Becca is also a DOT-certified medical examiner. She is passionate about providing
high-quality, compassionate, and empathetic care to her patients.
Wendy Peck, MPAS, PA-C
Ms. Peck grew up on the Mainline, originally from Wayne, PA. She graduated from Loyola University in 2006 with a
Bachelor of Science after studying a year abroad at Newcastle University in the UK. Before obtaining her Master's,
Ms. Peck volunteered in both South Africa and Guatemala, helping underserved communities. In 2014, she graduated
from Seton Hill University with a Master's in Physician Assistant Studies. Wendy worked at the Philadelphia Hand
Center until 2015, where she honed her skills in Orthopedics/Hand Surgery. She is also a DOT-certified medical
examiner. From 2015 until the present, she has decided to concentrate in treating people in Urgent Care and is happy
to be part of the AFC Family. She currently lives in Downingtown, PA, with her Husband and two daughters.
Julie Rich, MSN, CRNP
Ms. Rich is thrilled to be a member of the AFC team. She was born and raised locally and is a graduate of the
Villanova University College of Nursing. She then went on to obtain a Master’s in Science from the University of
Pennsylvania. She has worked in emergency medicine for the past 22 years, but will often say she has been working in
this field since she was 3 years old. In the early 1970s, her father helped design the NYC EMS system, and he would
take her on calls. Julie is passionate about patient advocacy and care coordination. Ms. Rich is also a
DOT-certified medical examiner. She is also an educator and is on the faculty at two area universities. She lives
locally in Chester County with her husband and two children.
Amanda Scholtyssek, CRNP
Amanda Scholtyssek is a certified registered nurse practitioner (CRNP) with extensive experience in both family and
emergency medicine. Originally from New Jersey, Amanda relocated to Hershey, PA, after earning a Bachelor of Science
in Nursing (BSN) from Misericordia University to work at Hershey Medical Center (HMC). There, Amanda worked as an ER
nurse for nearly five years before graduating from Widener University with her MSN, becoming a CRNP in 2016,
focusing on family and emergency care. She also completed a one-year fellowship in emergency medicine at HMC,
further enhancing her expertise. After meeting her husband, Amanda moved to the surrounding area and began working
at the Paoli Emergency Department. Later, due to the birth of twin daughters, she transitioned to urgent care,
balancing her professional career with family life. Ms. Scholtyssek is also a DOT-certified medical examiner.
Outside of work, Amanda enjoys spending quality time outdoors with her husband, daughters, and their dog, Charles.
Amy Valle, PA-C
Amy Valle brings over 25 years of experience as a Physician Assistant (PA), with a strong foundation in sports
medicine and acute care. She began her medical journey as an athletic trainer, earning a Bachelor of Science in
Athletic Training/Sports Medicine from West Chester University, where she also played lacrosse. After graduation,
she returned to her hometown in Maryland and worked in orthopedics for five years. During this time, she volunteered
at Maryland Shock Trauma, which inspired her to further her medical education. She completed her PA studies in 2000
and has since dedicated her career to acute medicine. Ms. Valle is also a DOT-certified medical examiner. Her
professional experience includes roles in Emergency Medicine, Occupational Health, and Dermatology. Since 2013, she
has focused on Urgent Care, seeking a better work-life balance to spend more time with her family. Outside of
medical practice, Amy enjoys swimming, running, playing pickleball, and reading.
