As of May 21, 2014, the Federal Motor Carrier Safety Administration (FMCSA) required that providers become certified medical examiners in order to perform DOT physicals. At AFC Urgent Care Rockville, all of our Providers are certified. DOT physicals require no appointment.
AFC Urgent Care Rockville is proud to provide affordable healthcare services and strives to keep costs low. We accept a wide range of insurance plans, both public/government and private/commercial.
Please call if you do not see your carrier listed as we are constantly updating the list of participating providers.
(Please file your claim with your insurance carrier prior to your visit)
*Certain Plans will not have any out of state/area benefit coverage. It is the patient’s responsibility to know and understand their benefits. If claims are denied due to limitations in coverage or if a patient has a deductible that has not been met, the patient will be responsible for the remaining balance of their claim. Patients’ insurance must be active at the time of the visit. If a claim is denied due to lapse in coverage, the patient will be responsible for the total amount of the visit.
Payment of applicable co-pay is required at the time of your visit.
A self-pay option is available for those patients who do not have insurance or choose not to process their visit through their insurance. Self-pay rates are discounted rates, with discounts ranging from 25-50%, depending upon the item. The Self-pay option requires payment in full on the date of service.
The cost of your visit will be determined by your specific insurance plan. We will verify your plan to determine your portion of responsibility. While we can’t confirm the exact total until your visit is complete, we can provide general information once insurance is verified.
We use insurance company tools (websites and phone systems) to confirm that your insurance is active and determine what portion of your visit is due at the time of service versus what will be billed to insurance.
It is usual and customary for patients to pay their portion at the time of service. This minimizes costly billing and helps keep healthcare costs low.
We accept cash, and all major credit cards including Visa, MasterCard, Discover, and American Express.
An outstanding balance comes from a missed payment from a previous visit. Our policy requires outstanding balances to be settled before treating new conditions.
Co-pay amounts are set by your insurance plan and employer. If your plan requires a co-pay, it must be paid at each visit.
A deductible is the out-of-pocket amount you must pay before your insurance starts covering costs. Once met, your insurance will begin to share expenses as outlined in your plan.
Insurance carriers calculate deductibles across all claims. Sometimes payments are still processing or have not yet updated. If your deductible is met in advance, any overpayment will be refunded to you.
Co-insurance is another cost-sharing mechanism required by some plans. Once your deductible and/or co-pay are satisfied, co-insurance represents the percentage of costs you are responsible for at the time of service.
