AFC Urgent Care Rockville offers official USCIS immigration medical exams for individuals applying for U.S. permanent residency, citizenship, or a Green Card. The immigration physical exam is a required step for green card applicants. Our providers are certified Civil Surgeons, authorized by the U.S. Citizenship and Immigration Services (USCIS) to complete and sign Form I-693.
We make the process simple and efficient, with most patients completing their exam in just two visits. Blood work, TB screening, and many required immunizations are available on-site as part of the comprehensive screening process. We also offer chest X-rays when medically necessary, following guidelines from the Centers for Disease Control and Prevention (CDC) for TB screening and disease control.
The immigration medical exam is a mandatory step for individuals applying for lawful permanent resident status in the United States, serving as a required medical examination and vaccination process for immigration purposes. It determines whether you meet the health requirements established by the Department of Homeland Security (DHS) and USCIS.
The completed results are documented on Form I-693, which is the official Report of Medical Examination and Vaccination Record. It is important to carefully complete all required forms and obtain the official report for submission with your immigration application.
The United States Citizenship and Immigration Services (USCIS) has established detailed requirements for the immigration medical examination, which is a vital part of the immigration process for most green card applicants. This exam must be performed by a USCIS-authorized civil surgeon and is designed to ensure that applicants meet the health standards necessary for entry or adjustment of status in the United States.
During the medical examination, the civil surgeon will conduct a thorough physical examination, review your medical history, and assess your vaccination record to confirm you meet all vaccination requirements. The exam also screens for any physical or mental disorders that could affect admissibility, as well as any health-related grounds that may impact your application. All findings, including the examination and vaccination record, are documented on Form I-693. This completed form, signed by the civil surgeon, must be submitted as part of your application to citizenship and immigration services.
The goal of the immigration medical exam is to determine if you have any medical conditions, including mental disorders, that could pose a public health risk or require special consideration during the immigration process. By following these USCIS requirements, you help ensure a smooth and timely review of your application.
You may walk in or schedule your first visit in advance. To avoid delays, please bring the following documents:
Civil surgeons are specially designated doctors who play a central role in the immigration medical exam process. Only civil surgeons authorized by USCIS can perform the required physical examination and complete the official Form I-693 for immigration purposes.
During your immigration medical exam, the civil surgeon will carefully review your medical history and vaccination record, making sure all required vaccines have been administered. They will also conduct a comprehensive physical examination and may order additional lab work, such as blood tests or chest x-rays, to check for communicable diseases or other medical conditions relevant to your application.
Civil surgeons are responsible for ensuring that your exam is thorough and meets all USCIS standards. They will document all findings, including any vaccinations given during your visit, on Form I-693. Once the exam is complete, the civil surgeon will seal the form for submission to immigration services, helping you move forward confidently in your immigration journey.
During your initial visit, our medical team will perform the required lab work:
A medical history review is also conducted during the first visit to ensure a comprehensive health evaluation, including assessment for tuberculosis and other conditions.
If any test results require further evaluation or treatment, we will contact you with next steps. If all results are normal, we will call you to schedule your medical exam with one of our Civil Surgeons
Your vaccine records will be reviewed to determine which vaccines, if any, are needed before moving forward. We offer translation services, if needed.
Your second visit is your official USCIS medical exam with a certified Civil Surgeon.

Once your immigration medical exam is completed, the civil surgeon will provide you with a sealed envelope containing your official exam results. It is important that you do not open this envelope, as it must remain sealed for submission to USCIS as part of your immigration application. You will also receive a copy of your exam results for your personal records.
USCIS will review your sealed exam results as part of the overall immigration process. If your exam reveals a medical condition that could affect your admissibility, such as a communicable disease or another health-related issue, you may be asked to provide additional documentation or undergo further evaluation. In some cases, if a medical condition is identified that would otherwise make you inadmissible, you may be eligible to apply for a waiver of inadmissibility. This process involves submitting further documentation and may require a more detailed review of your medical condition.
After USCIS reviews your completed exam and any additional information, they will notify you of their decision regarding your application. By following all instructions and submitting your sealed envelope promptly, you help ensure that your immigration process continues smoothly.
