If you are applying for permanent residence or a green card in the United States, you will need an immigration physical. As per the requirements of the The United States Citizenship and Immigration Services (USCIS) and the Department of Homeland Security (DHS), all immigration physicals must be completed by a civil surgeon, or a doctor who has been certified by USCIS. What Is An Immigration Physical?
An immigration physical, also known as an immigration medical examination or immigration medical screening, is a medical assessment required by the United States for individuals seeking to immigrate or obtain a visa for an extended stay. These physicals have specific requirements and can only be performed by civil surgeons, or doctors such as Dr. Clarke, who have undergone training and certification by the USCIS.
Who Are Immigration Physicals For?
People who are planning to move to the United States for an extended period, either permanently or on a green card, may be required to undergo an immigration medical examination. The purpose of the immigration physical is to assess the health status of the applicant and to ensure that they do not have any communicable diseases or conditions that could pose a public health risk to America.
What To Bring
In order to provide you with accurate information, make sure to bring the most complete and up-to-date documentation to your appointment possible. Please bring with you the following:
- Pre-completed I-693 Form (Report of Medical Examination and Vaccination Record)
- Valid government-issued ID or passport
- TB skin test results within the last year, including any previous positive tests
- Recent chest x-rays
- Vaccination or immunization records
If you don’t have your vaccination records, we can perform a blood test called a vaccine blood titer for proof of immunity.
What To Expect At Your Immigration Physical
An immigration physical will consist of a medical history review and physical examination. Dr. Clarke will order chest X-ray and blood tests, and discuss your previous vaccinations, your history of chickenpox, tuberculosis and any other existing health conditions. Dr. Clarke is also able to administer any vaccinations or blood tests you may need, including the following:
- Gonorrhea urine test
- Syphilis blood test
- Flu vaccine
- Diptheria vaccine
- Pertussis vaccine
- Mumps vaccine
- Rubella vaccine
- Meningococcal vaccine
- Pneumococcal vaccine
- Measles vaccine
- Tetanus vaccine

- Hepatitis A and Hepatitis B vaccines
- Varicella vaccine
At the conclusion of your appointment, Dr. Clarke will fill out the remainder of your I-693 Form and place it in a sealed envelope. Send the unopened envelope to the USCIS per their instructions. It is important that you do not alter the envelope in any way, or the USCIS will not accept it.
Will My Health Insurance Cover It?
Unfortunately, most insurance carriers do not cover immigration physicals. We recommend filing the necessary paperwork with your insurance provider regardless, and we can provide everything you need for this at your appointment.
