Certified Immigration Physicals
AFC Urgent Care Suffolk offers immigration physicals to patients applying for green cards or permanent residence in
the United States. These exams meet the specific requirements of The
United States Citizenship and Immigration Services (USCIS) and the Department
of Homeland Security (DHS). They can only be completed by a designated civil surgeon, or a doctor who has
received certification by USCIS. At AFC Urgent Care Suffolk, Dr. Clarke is a civil surgeon who is able to provide
in-house immigration physicals.
What Is An Immigration Physical?
Per the direction of the USCIS, all people applying for a green card or for permanent residence in the United
States must undergo an immigration physical, which consists of a medical examination and a physical examination.
They aim to promote immigrant health while preventing infectious diseases from entering the U.S. The physical must
be performed by a civil surgeon, or doctor that has been certified by USCIS. Additionally, these physicals can only
be conducted in person, not virtually.
Who Needs An Immigration Physical?
All people applying for a green card or permanent immigration status must receive this examination. In addition to
evaluating your overall physical and mental health, the examination tests for certain infectious diseases such as
syphilis, gonorrhea and tuberculosis. These conditions are considered inadmissible and people with them will either
not be permitted to enter the U.S. or they must wait to enter once the condition has been resolved in quarantine.
What To Expect
During the exam, our certified civil surgeon, Dr. Clarke, will review your medical history, immunization records
and physical health through X-rays, blood tests and observation. If you do not have access to your vaccination
records, we can perform a blood test to prove your immunizations. We can offer the following during your
appointment, however, you should call ahead to confirm the availability of these tests and vaccines.

- Pertussis vaccine
- Meningococcal vaccine
- Tetanus vaccine
- Polio vaccine
- Rotavirus vaccine
- Hepatitis B vaccines
What To Bring To Your Immigration Physical:
In order to ensure your appointment goes smoothly, please bring the following:
- Pre-completed I-693 Form
(Report of Medical Examination and Vaccination Record)
- Valid driver’s license or passport
- Vaccination or immunization records
At the conclusion of your examination, Dr. Clarke will fill out the rest of the 1-693 form and place it in a sealed
envelope for you to send to USCIS. We will also provide you with a copy of the form for your records. Please note
that USCIS will not accept the envelope if it is opened in any way.
Health Insurance Coverage And Immigration Physicals
Most health insurance plans do not cover immigration physicals. For this reason, we require payment for your
examination upfront. Our team is happy to supply you with the proper documentation to submit a claim to your
insurance cover, but we can not guarantee reimbursement.
