Before You Visit
Here are a few things you can do to prepare for your upcoming immigration physical:
- Use our online portal to save
your spot or call our office if you know what time you’d like to stop by. We want
to make sure that there is plenty of time to complete your examination.
- After registering online, you’ll receive an email containing a link to the I-693 form.
To help you save time during your first visit, complete the form electronically.
- Gather all your documentation including vaccination records, proof of tuberculosis
treatment (if applicable), and a government issued ID.
- Need an interpreter? A friend or family member 18 or older can come along to translate.
For $175 you can also request a translator at least 48 hours in advance from AFC
Orchards.
- Dress comfortably in loose clothing for full mobility during the physical exam.
- Prepare for blood work by drinking plenty of water beforehand.
- If applicable, translate documents into English and don’t forget your children’s
documents if they are also immigrating with you.
Visit 1: Paperwork and Initial Tests
Your initial visit will mostly consist of paperwork and tests. Plan to arrive 30 minutes prior to your
reservation for faster care. We’ll review the details of your immigration application and verify that all
necessary documents are present. An AFC team member will then collect blood samples for age-specific tests.
A crucial piece of your immigration application is the Form I-693, that’s why our providers will document
everything on that form.
Key items to bring to your first AFC Orchards visit:
- Government-issued ID (with photo) for yourself and any children immigrating with you.
- Vaccination records.
- Payment (cash or credit card).
- If applicable:
- Present any medical records you have (hospital discharge summaries, specialist reports or
records of past treatments).
- Previous chest X-rays or other relevant diagnostic tests.

- Any correspondence with the USCIS you’ve received.
- Ensure all documents are translated to English.
Visit 2: Civil Surgeon Exam
About a week after your initial consultation, you’ll visit AFC for a second appointment with a USCIS-approved
doctor, known as a civil surgeon. These are certified individuals who conduct immigration physicals. The
civil surgeon will begin by carefully reviewing your medical history. Past health conditions, current
medications and prior surgeries may be discussed.
Following the review, there will be an all-inclusive physical exam to assess your overall health (heart,
lungs, reflexes, and general well-being). Based on your medical history and the physical evaluation –
additional vaccinations, chest X-rays, blood tests or mental health evaluations, may be recommended by the
surgeon. After finalizing everything – including any recommended tests for vaccinations – the civil surgeon
will complete your Form I-693. The official I-693 will be given to you in a sealed envelope, along with a
copy for personal records.
