AFC Mt Vernon, NY Immigration Medical Services
Immigration Physicals
The United States Citizenship and Immigration Services (USCIS) requires a medical examination process. This
comprehensive assessment can take up to three visits with all services performed on site. The USCIS requires
these services be rendered by a certified civil surgeon, not one's primary care physician, for all long-term
visa, refugee, and adjustment of status applicants. Immigration physicals are utilized to maintain and ensure
the health and mental status of persons applying for citizenship.
AFC Urgent Care in Mount Vernon, NY has fully trained and certified medical civil surgeons on
staff.
We handle all aspects of your immigration physical, including, but not limited to:
comprehensive health screening
phlebotomy and titers
required vaccinations
radiology/x-ray, if applicable
referral to outside provider if necessary
The medical exam portion of the immigration process can take two to three visits, so it helps to know what to
expect and what you need to bring. It may also be helpful to have a translator with you, if needed. To help
you and your family make as few trips as possible, here's what to expect for each of your visits:
VISIT ONE
We accept walk-ins for the first visit with a scheduled follow-up visit.
Please bring a valid, government-issued photo I.D. or a passport (passport, driver's license, employment
authorization, learner's permit, state ID).
Applicants under 14 years old are permitted to have other proof of identity that shows name, date and place
of birth, parents' full names and other identifying information (birth certificate; school ID with child's
name, if available)
Please bring all vaccine records (must be professionally translated in English) so we know if you need
additional immunizations. Don’t worry– we offer many of the required vaccines at our clinics, if you
need them; an additional charge may apply. To schedule a visit with one of our civil surgeon doctors, you
must have records of all the required vaccines.
Based on age and certain requirements, three blood tests can be done during the first visit – QuantiFERON
(TB Test), Syphilis, and Gonorrhea screening.
VISIT TWO
We will schedule an appointment for the second visit.
If the results from all tests are negative, you will be able to schedule your immigration medical exam with
one of our civil surgeons and expedite the completion process.
If results from the tests are abnormal or positive, you may require further treatment. If this is necessary,
our team will give you the details and referrals you may need to guide you through the process.
Our team will schedule an appointment for your third visit once all requirements are met.
VISIT THREE
One of our civil surgeons will perform your immigration medical exam.
Your I-693 paperwork will be completed and sealed for you to submit to USCIS (Do not open package). You will
also receive a copy for your records. The medical portion of the immigration process is now complete!
Congratulations!
