U.S. Citizenship and Immigration Services (USCIS) Physical
Welcome to AFC Urgent Care Holland. We are privileged to play a part in your process of earning your
green card. We have included some information below to make this process go as smoothly as possible for you. If
you have any questions, please ask any one of our staff members, as it is our goal to make you feel as comfortable
as possible.
Your USCIS physical has three requirements:
- Vaccinations
- Lab work
- Physical exam
Vaccinations
It is best for you to ensure that all the required vaccinations are current prior to visiting for your lab work. If you are
unable to obtain your vaccinations prior to the visit for your lab work, you must have your vaccinations prior to the
physical exam. We cannot proceed with the physical exam until we have evidence that you have been completely
vaccination. Prices for vaccinations will range greatly, depending upon whether you have insurance, Medicaid, or
are self-paying. Ottawa County Department of Public Health offers a sliding scale for payment, meaning if you have
little to no income, you may pay $0 to very little. The following are the required vaccinations:
- Mumps, Measles, and Rubella (MMR)
- Tetanus, diphtheria and pertussis (TDAP)
- Haemophilus influenzae type B (Hib)
- Hepatitis B
- Meningococcal disease (MenACWY)
- Pneumococcal disease
- Polio
- Seasonal infuenza (September through April)
- Hepatitis A
- Rotavirus
- Varicella
You can obtain your vaccinations at your local pharmacy or health department. Below are some locations near our

Lab work
During your lab work appointment, we will collect blood and urine samples for Gonorrhea, Syphilis, and
Tuberculosis. The samples will be sent to an outside third-party laboratory. The price of the labs changes
occasionally, however estimate about $115.00 for the lab work.
Physical exam
Please remember to bring your vaccination or immunization card to the appointment for your physical exam. We will
not be able to proceed without evidence of vaccination. It is also helpful to bring form I-693 with parts 1 through 4
completed. The civil surgeon will complete the remaining sections. If possible, please bring copies of any medical
history, x-rays, or letters from your regular doctor outlining your treatment plan. The cost of the physical exam is
$400.
The civil surgeon will conduct the physical exam according to parts 5 through 8 of Form I-693. Parts 5 through 8 are
to only be completed by the civil surgeon. The results will be placed in a sealed envelope and given to you to bring
to your immigration interview. DO NOT break the seal. If the seal is broken, the process must be repeated, and we
will need to bill accordingly.
Collection of fees and scheduling
Fees for the labs and the physical will be collected during the first visit before the labs are drawn.
Vaccinations will be billed by the pharmacy or the health department.
The lab work and the physical will be scheduled in two appointments at AFC Urgent Care Holland. The lab work
should be scheduled at least 5 days prior to the date of your physical. Both appointments need to be scheduled in
advance.
Your doctor will order a chest X-ray and blood tests. Your physical and mental disposition will be assessed through X-rays, blood tests, and diagnostic observation. In addition to your Form I-693, bring your medical and vaccination records for the doctor. If you don’t have proof of your vaccinations, you can request a blood test to prove immunization.
