Daniel Facer, PA-C
Daniel is a graduate of Duke University, holding a Master of Health Sciences in Physician Assistant Studies. He brings extensive experience as a primary care provider, serving patients across all age groups. With an additional degree in Clinical Social Work, Daniel offers a unique blend of medical expertise and deep interpersonal insight. His approach to care is rooted in empathy, compassion, and a genuine commitment to patient well-being.

Jonathan Torres, PA-C
Jonathan earned his Master of Health Sciences in Physician Assistant Studies from Drexel University. With a strong background in Emergency Medicine and Urgent Care, he brings critical expertise to fast-paced clinical settings. Jonathan also has a solid foundation in Science and Biology, paired with a keen understanding of modern medical testing technologies. Known for his attentive and thoughtful demeanor, he ensures that patients feel heard, respected, and well cared for.

Meet Brandon Pervis

Brandon Pervis is a longtime Connecticut resident and a proud graduate of Sacred Heart University in Fairfield, where he earned his Master of Science in Nursing as a Family Nurse Practitioner. He brings extensive experience in caring for geriatric patients and has developed educational programs focused on chronic condition management, medication adherence, and preventive health strategies for patients and their families.

Brandon is known for his warm and approachable demeanor, making him easy to talk to and deeply appreciated by both patients and staff. His compassionate care and commitment to education contribute greatly to the well-being of those he serves.

Meet Noman Zafar

Noman Zafar is a dedicated healthcare professional with a strong academic and clinical background. He graduated Magna Cum Laude from Central Connecticut State University with a Bachelor of Science in Biomolecular Sciences and went on to earn his Master of Science in Physician Assistant Studies from the University of Bridgeport.

In addition to his PA credentials, Noman is also a certified X-ray technician. He has extensive experience working in both hospital and urgent care settings, where he has provided compassionate care to a wide range of patients — from pediatrics to adults and geriatrics.

A longtime Connecticut resident, Noman has deep ties to the state and a strong commitment to serving the local community with excellence and empathy.

Kristen Kinslow, PA-C

Kristen Kinslow is a lifelong Connecticut resident with deep family and community roots in Fairfield County. She earned her Bachelor of Science from the University of Scranton and her Master of Science in Physician Assistant Studies from the Philadelphia College of Osteopathic Medicine.

Kristen brings extensive experience in both Family Medicine and Urgent Care, and has been a valued member of the AFC team for over two years. Known for her dedication, kindness, and thorough approach to patient care, Kristen is highly regarded by both staff and patients. Her warm and amiable personality makes her especially effective in treating patients of all ages, and she has a special affinity for caring for children and the elderly.

Outside of work, Kristen is a devoted wife and mother to two young sons. She is actively involved in their school and religious communities, serving as a room parent and frequently volunteering at school functions and local civic events.
