What is a DOT Physical?

A DOT physical is a federally mandated medical examination required by the Federal Motor Carrier Safety Administration (FMCSA) for commercial motor vehicle operators. It is governed by 49 CFR Part 391 and must be conducted by a medical examiner listed in the FMCSA National Registry.

The exam confirms that a driver is physically and mentally fit to safely operate a commercial vehicle. Upon passing, you receive a Medical Examiner's Certificate (Form MCSA-5876) — which you are required to carry while operating a commercial vehicle.

Who Needs a DOT Physical?

You need a DOT physical if you operate a vehicle that meets any of the following:

  • Gross vehicle weight rating or gross combination weight rating over 10,001 lbs
  • Designed to transport 16 or more passengers (including the driver) without compensation
  • Designed to transport 9 or more passengers for compensation
  • Used to transport hazardous materials in quantities requiring placarding

This applies to most CDL holders and operators of semi-trucks, tractor-trailers, dump trucks, tankers, flatbeds, buses, and other commercial vehicles operating in and around Houston.

How Long Does Certification Last?

A standard DOT Medical Examiner's Certificate is valid for up to 24 months. Drivers with certain conditions like hypertension may receive a shorter certification period and require more frequent exams. Your AFC provider will advise based on your individual health history.

What to Bring to Your DOT Physical

Bring the following to your appointment. Missing items may delay your exam:

  • Valid government-issued photo ID or your Commercial Driver's License (CDL)
  • Completed FMCSA Medical Examination Report (Form MCSA-5875). You can fill this out at the clinic, but completing it in advance shortens your visit
  • Full list of current medications and dosages including prescriptions, over-the-counter, and supplements
  • Hearing aids, glasses or contact lenses if you use them
  • Documentation of any previously issued FMCSA waivers or exemptions
  • Medical records or specialist letters for chronic conditions (diabetes, heart disease, sleep apnea)
  • Payment method (cash, credit, or debit)

FAQs About DOT Physicals

Most DOT physicals at AFC take 30 to 45 minutes from check-in to certificate. Completing Form MCSA-5875 and your medication list before you arrive shortens your visit.

No. All AFC Houston locations accept walk-ins for DOT physicals. Online scheduling is available if you want to save your spot in advance.

$65 for all locations except AFC Beaumont, where exams are $95.

Generally, no. DOT physicals are an occupational service and are typically not covered by health insurance. Most drivers pay out of pocket. If your employer covers the cost, bring documentation. Ask your AFC location about accepted forms of payment.

If the examiner identifies a condition that affects certification, they will explain your options. Many drivers can return with specialist documentation, receive a conditional certification, or apply for an exemption program.

No. The urinalysis included in the DOT physical screens is for medical conditions. It is not a drug or alcohol test. If your employer requires a DOT-mandated drug screen under 49 CFR Part 40, that is a separate test. AFC also offers DOT drug testing.

In most cases, every 24 months. Drivers with conditions requiring closer monitoring may receive a 12-month or shorter certificate. You will need to recertify before your certificate expires to remain in compliance.

No. Your CDL is used for identification verification only. You must complete the FMCSA Medical Examination Report (Form MCSA-5875) as part of the exam. You can fill it out at the clinic or download it in advance from fmcsa.dot.gov.
