Our comprehensive medical tests encompass TB testing, urine analysis, vaccinations, and overall physical wellness assessments. We prioritize convenience and flexibility for our clients, offering both scheduled appointments via our website and accommodating Walk-Ins. Our dedicated staff is on hand to guide you through the required documentation for your visit. In certain instances, a follow-up appointment may be necessary, particularly if initial test results are inconclusive and require additional analysis. Rest assured, the results of your immigration medical exam, communicated to USCIS through form I-693, will be thoroughly reviewed, and any significant medical findings will be promptly disclosed to you.

What you need to bring

Patients are advised to bring the following to their appointment/WALK-IN:

• Government issued picture ID such as a driver’s license or passport (if physical is for a child, the child will need photo ID as well. Parent ID will not be accepted)
• Vaccine/Immunization records (if records are in a different language they will need to be translated into English): www.uscis.gov/i-693
• Hospital or clinic records, including proof of prior treatment for tuberculosis or other infectious diseases.
• Prior chest x-rays and/or other diagnostic tests
• Any mental health records.
• Immigration Physical Form I-693: you may complete this online in the following link: https://forms.MedFiller.com/Start/72ydsn31k6ll9ue6 or you may fill one out here at the clinic.
• Payment (we accept cash or credit/debit. We do not accept checks).

All Exams include:

• Physical Examination by Civil Surgeon
• Gonorrhea Test (18 to 24 years old)
• RPR (Syphilis) Test
• TB Test (via QuantiFERON Gold)

Cost Per Person (paid during initial visit): Adults (over 15) – $199

• Chest X-Ray - $85
• TDAP – $100
• MMR – $120
• HEP A – $90
• HEP B (each injection) – $145
• Varicella – $230
• Flu (October 1- March 30) – $60
• Titers for Hep B, MMR, Varicella. $100

If a physical needs to be repeated because it was not submitted to USCIS within 60 days of the date it was completed and signed by the civil surgeon, the pricing is:

• Physical examination with paperwork- $150
• Labs, Vaccines & Services- if any of these have expired will be additional fee.
