Every applicant must bring a valid form of government-issued ID. USCIS needs to verify your identity with the civil surgeon, and without proper ID, the exam cannot legally be completed. Most people bring a passport, but these options are also accepted:
- Passport

- Driver’s license
- National ID from your home country
- Work permit (EAD) in some cases
Make sure your ID is not expired. If your passport is the only ID you have, bring it even if it’s from your home country.
Your Completed (But Unsigned) I-693 Form
You can download the I-693 form directly from the USCIS website. You’re allowed to fill out the first section, but do not sign it before your appointment. USCIS requires that your signature be witnessed by the civil surgeon. If you sign it early, the form becomes invalid and you’ll have to redo it.
