Please verify in advance if you need any specific documentation for your flight and notify us when you are seen. ***IMPORTANT: We encourage you to file your own claim with insurance for potential reimbursement. We can provide you with an itemized receipt with proper testing codes for insurance reimbursement at the time of service to aid in that process.
