Do I Need A DOT Physical Exam?

A Department of Transportation (DOT) physical is a medical exam that confirms whether or not commercial drivers are healthy enough to safely operate vehicles that carry a gross combination weight of 10,000 pounds or more. If you’re a commercial driver, or plan on becoming one in the future, a DOT physical is required by law to ensure safety for you and those around you.

Whether it’s your first DOT physical exam, or you simply require a renewal, AFC Urgent Care Louisville is there for you. Visit the clinic today on a walk-in basis. Read more about how to prepare for the appointment and what to expect during the examination.

How Do I Prepare For the Exam?

If you wish to save time during the appointment, you can fill out the driver's portion of the exam at home. The form can be found here. Otherwise, all necessary paperwork will be provided by your healthcare provider. Upon arrival to the physical, wear loose, comfortable clothing. Don’t use the restroom just before your appointment, as you will need to give a small urine sample for a kidney screen.

In order to be best prepared for the examination and any questions your provider may ask, please bring the following:

  • Your valid driver’s license and /or other photo ID.
  • Eyeglasses, contact lenses, and /or hearing aids if applicable.
  • A list of medications, and the name and contact information of the prescribing doctor.
  • A record of your blood sugar readings and your most recent Hgb A1C if you have diabetes.
  • A record from your CPAP machine showing proper use for at least 90 days if you have sleep apnea.
  • A letter from your cardiologist indicating you're safe to drive a DOT physical if you have heart issues.

If you have any other medical conditions, please contact us at (720) 961-9700 to find out more information about what to bring to the exam.

What to Expect During the Examination:

During the exam, your urgent care medical provider will review your health history with you, asking a series of questions about any health concerns you may have. Your provider will also conduct a thorough physical examination, which includes a urine screen (dip-stick test) to check for general health levels and the onset of possible disqualifying medical conditions.

How Long is a DOT Medical Card Good For?

DOT medical cards are valid up to 2 years – if you meet all the FMCSA DOT regulations for commercial driver fitness. If periodic monitoring is required for certain health conditions, your DOT medical card will be valid for a more limited period, usually 6 months to a year.

If you have recently been diagnosed with a serious condition, a medical professional at AFC Urgent Care Louisville will usually issue your card for a 1 to 3 month window to monitor if your condition prevents you from safely operating a commercial vehicle. Upon completion of the exam, your urgent care provider will supply more information about whether or not you qualify and why.
